Get a Library Card

Version en español

New library cards are still available during the building closure! Please complete our online library card application, and we will contact you within a few days.

If you need assistance accessing Overdrive, LinkCAT, or other online resources during the library closure, please call us at (608) 873-6281 or email us at storef@stolib.org.  You can also use our Contact Form. This includes help with lost and expired library cards.

Library cards are available free of charge to Wisconsin residents. Your library card can be used at any public library in the South Central Library System.

When the library reopens, you can download a library card application. Bring that form to the library along with a copy of your photo ID and proof of your current address (within the last 30 days). Please contact us for details about acceptable forms of ID and proof of address. If you are signing up multiple members of your household at the same time, use our Family library card application (2 pages).

A valid library card must be presented to check out. (See the Patron Registration policy for details).

Parents/custodial guardian must sign the application form for children under the age of 16.

Please inform the library about any changes of address, phone number or email address.

If your library card is lost, report it as soon as possible. Call 608-873-6281.
