To obtain a library card, complete a library card application form and bring it along
with photo I.D. that includes proof of current residence.

A driver’s license with current address is the easiest way to apply.

If your address is not correct on the I.D. you will also need to bring one of the following:

Checkbook with name and current address printed the checks
OR
A current lease with your name and address on it
OR
A piece of current mail that includes the envelope with your name, current address
and a postmark dated within the last 30 days

Library card application forms are available at the library or print it out and take the completed form to the library.

The first card issued by Stoughton Public Library is free.
Replacement cards cost $3.00 and limited to 3 replacements per year.

Patrons age 17 and under are required to have the signature of their legal guardian.

A handwritten address correction label on a driver's license is not proof of current address.

If your identification has a P.O. Box or business address, you still must show proof of your current home address.
